contification paper: formatting

I guess that I'm arguing (weakly) for admitting up front that MLton's IL
should handle mutually recursive continuations, and that it is a historical
accident that it doesn't.  Thus the fact that it doesn't cost us too much is
nice, but that you wouldn't do things that way now.
Any way, don't worry about it very much.  It was just something that I never
was comfortable with when contification first started getting discussed.
